Perm in process, H1 6yrs question

My cousin who is nearing 6yrs of H1 in few months, and his PERM is in process (from last 2.5 months).

If he do not get his PERM approval before the expiry, is he eligible to apply for H1 extension?

No. Not unless the perm was applied 365 days before max out date 

your cousin has 2 options after max out date - leave the country or convert to b visa if they does not want to leave country or convert to student visa if they have an admission etc. ready

Ok, if it gets approved in 6 months after his visa expiry - will he be able to transfer his status to H1?

Not recommended before 140 approval and only after 6 months of approval

They can come back to h1 after 

perm, i140 and h1 is approved 

no need of stamping - just h1 approval is enough 

attoryney should revoke b visa petition after h1 is approved 

they should not work or get paid while on business visa
