Ds160 question for H4 visa

Applying h4 for my child . Both my wife and I working here and at the time of visa appointment we are planning to visit India. In that case what should i answer for these questions. Thanks for your time.

Is your father in the US?
Is your mother in the US?

Did anyone assist you in filling out this application?  Should i say yes since i am submitting on behalf of my child who is under 14.
