They are referring to visas like B1, B2 and other very short term visas who intentionally over stay and stay back in the country for illegal alien.

For H1/L1, when extension is filed, the beneficiary still have 240 days to stay in US post the current I-94 expiry. This is legal this valid, may not be changed, not easy to change.

If you stay past your I-94 expiration date, you are considered to be "out of status" and have overstayed your visa, which can lead to serious immigration consequences like being barred from re-entering the US for a period of time.
