Khameini is not dead - yet. I am fairly certain Israel + USA could eliminate Khameini if they wanted to. After all Trump 1.0 eliminated Soleimani who was essentially considered No. 2 in Iran in 2020.

The ramifications of such an elimination is something perhaps the administration is weighing.

Iran is hopelessly compromised from the inside. The fact that Mossad picked out so many of their top tier military leaders clearly shows Mossad has penetrated pretty deep into the Iranian civil society - sections which are completely disillusioned with the Iranian regime. Imagine killing Ismail Haniyeh - the leader of Hamas right in the capital of Iran in Tehran when inside his guestroom. 

Think about the number of top echelon military commanders and nuclear scientists that Mossad was able to kill. It simply could not have happened without Mossad developing assets within Iran.
