hello guys 

need your suggestions. I have high uric acid levels (9.5 undi naaku instead of max 7.2)

tablets eskuntunna. 2 weeks back doctor daggara poina...anti inflamatory tablets ichindi....taggindi....

Yearly checkups lo they said no issue with thyroid, kidney, etc etc

mandhu manesa for 3 weeks
mukka kuda tinaledu (only chicken 2 times tinnanu in 3 weeks)
regular ga 3-4 times badminton aadutha in a week. gym ki 2 days potha in a week

malli gout ochindi ivala.....jeevitham mida virakti ostundi

 

ofcourse doctor daggara potha repo ellundo.....but any good suggestions from you guys?

I will explain it clearly forget whatever everyone said, I am a doctor so follow this explanation carefully 

Gout is due to excess of uric acid when sufficient PH is met is deposited as uric acid crystals in the joints

Uric acid is produced from purine metabolism only 1/3rd of the purines in the body is from dietary source approximately 400mg/24 hours and 2/3rd is derived from body own metabolism mostly during recycling of RNA and DNA in our body which is approximately 800mg/24hours, so now you can understand by this diet is not the major factor ,the major factor is your body producing uric acid , some may produce more uric acid due to mutations which are not fully understood but one such condition is inherited condition called lesch nyhan syndrome 

Now coming to excretion of uric acid 2/3rd of it is excreted in urine that is 800mg/ day and 1/3rd through stools appx 400mg/day

Some of the mutations can also cause increased reabsorbtion of uric acid back into the body through kidneys

So for increase in uric acid levels the problem can be any where in above mentioned processes 

In over 90% of cases the cause of increased uric acid in the body is reduced uric acid excretion by kidneys which is genetically determined 

So from all this you can understand the only thing in your hand is diet control but this is not a major contributing factor of the problem

So reduce the intake of purine rich foods such as sea foods, red meat and fructose rich foods such as processed bakery items and soft drinks should be limited and alcohol especially beer should be avoided and reduction of weight is also suggested by regular exercise and increase the intake of vitamin c rich foods such as citrus fruits

Disclaimer don't except any drastic change in improvement after dietary changes as uric acid is already deposited in your joints 

Now coming to the most important part the pharmacological treatment 

During gout episode we usually give colchicine 0.5mg twice or thrice daily ,oral NSAIDS like indomethacin can also be given but we prefer colchicine here

The drug you are taking is allopurinol which increases the excretion of uric acid in the urine should not be given during the period of the gout flare but after the episode subsides to prevent the recurrence so should not be given during initial episode , we start with initial dose of 100mg daily

Allopurinol initially causes dissolution of uric acid crystals already deposited so it temporarily causes increase of uric acid levels in the body so initially after start of treatment of allopurinol you get more flareups of gout but gradually the uric acid levels will stabilise and episodes will reduce so stay strong and also the initial dose in your case is 300mg which is high in my view so your fluctuations of uric acid is more than expected so more gout flare, so be patient it will work

The dose of allopurinol can be increased by 100mg every 4 weeks until the uric acid levels in the body stabilises

After all this if there is no sufficient improvement then there is febuxostat which can replace allopurinol 

Don't stress too much on dietary changes , but try as much possible, highly restrictive diet not necessary 

Hope this helps, I know it's difficult now but stay strong the condition will improve but remember this is long term battle
