mi fren ni diet habits ni monitor chesukomanu mundu. ask him to drink lots of fluids(not alcholic) daily.

Take your friend to the hospital. It might be appendix. Not necessarily the pain should be on right side to confirm to be appendix. Better take him right away.

Regarding insurance:
It is a federal law that no hospital can turn you away because of inability to pay. They also must treat you until you are stable. They do not have to cure you, but treat you enough so that you can leave the ER and not be in danger, So you can receive free medical care.

Some hospitals have internal "charity care" or "financial aid", but not all. If you have little money and are uninsured or under-insured they can cover your costs (doctor's fees are usually not covered),make use of it.

Hope your friend feels better and learn a valuable lessen and get insurance as soon as possible.
