TO UNIVERSITY

1.copy of I-765 form
2.copy of visa,passport,I-94 bothsides
3.copy of current EAD card
4.copies of all previously issued i-20's
5.OPT EXTENSION form your univ website
6.copy of degree certificate
7.copy of employment letter with e-verified num on it.

if you submit all the above to your ISO..they will issue a new i-20.



TO USCIS

1. Completed I-765 form (from USCIS.gov website) - Must be original - no copies
2. Copy of Passport, Visa and I-94 (both sides)
3. Copy of newly issued OPT STEM Extension I-20,
4. Copy of current EAD card (both sides)
5. Copy of employment offer letter with E-verify num on it.
6. Copy of degree certificate from  University .
7. Copies of all previously issued I-20s, including those from other
universities if you are atrasfer student.
8. Two identical passport-style photographs
9. $380 fee (check or Money Order, payable to “U.S. Department of
Homeland Security”) [color=red]fee check chesukondi oka sari i dont know exactly [/color]
