I , V.Kalpalatha W/o. Late V.Siva Kali Prasad ,aged 44
years , submit that ;
1. I had one son and one daughter bearing the responsibility of
bringing them up and creating educational facilities.
2. My only son,V.S.B.S.Sarath Chandra, passed 10th class in first
class held in March 2008 ,intermediate in year March 2010 got first
class and now studying B.Tech.2nd year in D.J.R.College of Engineering
and Technology,Gudavalli,Krishna district during 2011-2012 (I.D.card
in attachment).
3. My son is suffering from which was detected during 10th class study
and he is undertaking medical treatment for the last 9 months in
Vijayawada and Hyderabad under the guidance and medical treatment of
expert doctors.(Documents are attached below which was given by
doctor).
Since the treatment is costly and my economic conditions don’t permit
me to meet the expenditure,I request the honourable and
philanthropic donors to extend financial help to State Bank of India,
