Help Needed Urgent About Going To India In Opt

Guys.. can u just help me with this issue. My opt period start in August 2011. I applied for 17 month OPT extension today(June 15 2012). I wanna go to India in December 2012. What all do i need?? about the letters and everything..... Please.. Urgent

Travel I20...and ur employer letter.....and valid F1 visa....on ur passport...
