Help Needed.. In Trouble Regarding Opt Extension

Hello everyone.. I applied for opt stem extension recently. On the I-765 form in the address column, i gave my friends employer address coz he can recieve it safe. But I got a notice to that address for a Biometrics appointment near that location. But I relocated to a different state to the client site. So i called the USCIS customer care and told them that I relocated and they asked me fill AR11 form for address change online. In that AR11 form, its asking me my past address. Now my question is whether to fill in the address of my last stay or to fill the one I used on I-765??
Please help me with this urgent. I am feeling very tensed about it..
