Follow these steps:

1.       First, take a money order for $91.20.

2.       Go to Fedex with  your address and the Passport center’s address and ask them to create a shipping label (with To address to the Consulate) and a return shipping label (with To address to your home)as well.  Your documents weight would be 0.2 lbs. This will give you tracking numbers that you will use later.

3.       Complete the online application@ https://passport.gov.in/nri/Online.do. You will need to take a print and affix Photo after submitting online.

4.       Download and Complete the Nationality Verification Form at http://www.immihelp.com/nri/indianpassport/nationality-status-verification-form.pdf. Affix a photo to it.

5.       Download and Complete the checklist @http://www.passport.blsindia-usa.com/pptchecklist/BLSUSPassportChecklist.pdf

6.       Get a copy of your I-797 and Driver’s License notarized. If you need your spouse’s name in the Spouse field of the passport, you need to get your Marriage certificate notarized as well.

7.       Additional three photpgraphs.

8.       Fill in the Passport Order form @ http://blsindianvisa.com/orderform/passport.php. Use the Fedex tracking numbers as the Airway Bill numbers in the form. Take a return envelope and affix the return shipping label to it.

9.       Put all the forms including the return shipping label in another envelope and affix the shipping label addresses to the consulate and send it.
