I think you need to file for name change in local court and then publish a notice in newspapers ..and then take all docs to Indian embassy and get teh name changed..simialrly apply for name change for all universities along with the doucments/proofs  and get certificates issued on new name

 

Or

 

Just add alias name in the passport with out all headaches ..she can continue to use the maiden last name for all other purposes like taxes,job etc..it will not effect in any way !
