Yes,,, DOB is a must .. and it should match your records.... please get it asap... and if there is any problems.. you have to get it fixed.. and now days meeseva is the only place it issues

Name should match with passport, correction vunte get it new .

 

ekkada enduku risk accept cheyakapothe bokka kada, still you have time 10-20 Rs kadithe duplicate estharu

 

also name on birth certificate should be in English lekpothe affidavit kavali
