help me Ds 160 Doubts

Hi Friends,

 

I am filling my DS 160 form, please advise me if you know about below questions 

 

1) have you ever been issued a us visa ?

I got F1 visa in 2008 and First H1B visa in 2013 ( approved for one year, didn't went for stamping), H1B extension in 2014. Which visa i have to choose for the above question ??

 

 

2) Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services?

 

My employer filed H1B on my behalf, should i mention about my employer or leave it as NO ??

brother... for 1: do you mean i need to give my F1 details or H1B details ??

 

for 2: my employer recently applied for green card in sep 2014. so do i mention my employer name ??
