you have to register above link and fill all details online (note that it is a govt. website)... at the end you will be  able to print/save the pdf.  take it to passport office nearby along with suggested annexures (bascially address proof, DOB proof etc... mostly aadhar, voter id, DL, 10th mark list etc should do)

after some day they will send a constable for ur verification check if u dont have any crime record then uu will reciveur passport
