ceelogreen Posted August 15, 2015 Report Posted August 15, 2015 no specific format.... whatever u put make sure you have the following things mentioned: Job title employment dates Full time or part time No.of work hours per week Job location Brief description of job duties performed technologies used ( relevant to the PERM requirements) Put the current contact details of the prev employer ( Address, Phone etc) AT the end put the name of the person whom u interacted with during recruitment in that prev comp/ friend or colleague

Some paystubs might also help..basically as many documents as possible that can prove.that you really worked for the employer...send them to your attorney and he can decide which ones to use...
