I-20 (just pages 1 & 3; make sure section #11 is signed) 3. I-901 SEVIS fee payment receipt (go to www.fmjfee.com to pay the $200 SEVIS fee) 4. Evidence of financial support. Past 4 months of bank statements or proof of a GTA/GRA/scholarship are acceptable (a letter from the bank with current balance is insufficient). 5. H-4’s I-94 (front and back) 6. H-4’s visa used to enter U.S. 7. H-4’s passport pages() showing name, photo, date of birth and expiration date 8. H-1B’s Approval Notice (Form I-797A) with I-94 attached at bottom 9. H-1B’s visa 10. H-1B’s passport page(s) showing name, photo, date of birth and expiration date 11. Proof H-1B still employed. e.g. current letter verifying H-1B is still employed in position as approved in I-797 or a current paystub
