I had a similar prob.. digital and all gurthu ledhu .. but name change vunde.. like 3 years back .. full dhum dhum chesar eseva lo.. aithe simple ga municipal office ki poyi oka 3k isthe .. 1 hour lo chesi pettadu .. ATB

But make sure nuvu ekkada ayite puttavo aaa dist/mandal meeseva lo ne submit cheyyaali

If I remember correct, nen aithe direct ga municipal office lo birth records enter Chese vaadi deggarike vella.. Pani chesi pettadu .. Eeseva and all badulu municipal office broker evarini aina matladukoni pani cheyinchuko.. As simple as that. Or follow the standard procedure.. :)
