parents can do... no need of our presence... parents ekkada vunte akkada cheyinchocchu...but send the copies of supporting  documents that u are mentioning in the affidavit to parents becoz some notaries may ask to verify just in case....

Not sure other person provided serves your purpose..if u don't have bcertificate then non availability plus that template works..in your case you need to justify about the last name expansion..

Per my knowledge here are the times u need to provide affidavits

No bccertificate : non availability plus affidavits I think ceelogreen template works

Late registration : registered after your 1 yr of birth template diffe

Name discrepancy : explaining how we use in india

Instead of both parents get one from uncle or relatives.. I can give u template for name expansion soon..

So are u going with correction or will be using existing bcertificate either way u have to submit affidavits based on the type..
