let say you (name as bablu) have a health insurance plan from BCBS with the terms as $1000(deductible), $5000(out-of-pocket), 20% co-insurance.

 

so now in 2015 february, You (named as bablu) went to urologist and found that bablu got STD disease, Ghannerhia disease. So doctor suggest Piles surgery. you went for surgery in 2015 march. surgery costs $100000.00

 

Now you have to pay first $1000 dollars (which is deductible). once you paid deductible, your 20% co-insurance kicks in. This means that you start paying until the amount reaches to $4000. so this means that you paid total $1000(deductible)+ $4000 (part of 20% co-insurance) = $5000 (which is out-of-pocket limit). after this payment, remaining $95000 will be paid by BCBS.
