mestri corporate group health insurance lo enroll ayithe ee forms mestri pamputada or insurance company vadu pamputada?

Taxfiling ki cpa ee statements 1095 adugutunnadu. Idi proof anta to show we had insurance in 2015 and this is strict requirement to file taxes anta. Last year emi adagale but this year nunde asking. How to get this and manaku edi applicable(A or B or C form?)
