what do you mean by group insurance? is it employer provided insurance? None of the insurance shouldn't exclude spouse by Working/Non working status...

This happens to me all the time. Call insuracne and request them to re-process.

Sorted out most of them man.. Insurance told that they didn't get diagnosis codes... and also address was not updated with the hospital .. in case labs deny your claim talk to hospital and ask them to send diagnosis codes
