The code/document must be your original work and not done by anyone else. Presentation format is not important. Even if it is a few different files which don’t work & screenshots, client ok with it as long as they understand how you thinks and designs/programs/test. Also, Please submit a brief description of your background particularly summarizing your last 2 years of work history. This is not your resume but this should illustrate specifically your role in the teams and projects and specific duties on a day to day basis.
