visa fee payment help

i am planning to pay visa fee today for h1(india--hyderabad). Do i need to carry passport along with me for the fee payment??

one more que, After filling ds160, i tried to check availabilty for slots, there is one column named 'Peteitoner" and for that it is asking for start date and end date. When i tried to enter

the details which is there in I797, it is not accepting. Any idea about this??
