For visiting & Seeing, I can recommend the following:
 
Bio Luminiscnet bay: We went to the one in Fajardo bay which is an hour from Condado.Make a reservation ahead of time, they book out quickly.
 
It is a 2 hr kayak from the bay and needs some energy. So plan accordingly. Get some mosquito repellant and leave most of the stuff in the car. Wear flipflops or something for almost naked-feet.
All the operators pretty much do the same, but I booked on http://www.kayakingpuertorico.com/
They have other tours as well which were highly reviewed.
 
For an all day experience, try to do the Culebra and Aquafari Culebra. 
This thing gets sold out fast as well. We made a reservation but that got cancelled due to choppy water. I do not have personal experience but was very excited.
To do this:
Start early morning. Drive to Fajardo (1 hr avg)
Take a ferry to Culebra (Island)
Then they will take you on a smaller boat to do kayaking and snorkeling to see Sea Turtles and other water animals. 
 
The rainforest in the San Juan area (El yunque) was pretty good too. 
We went there for few hours, did a 4 miles round trip hike (La Mina trail). At the end of the hike is a small water fall with small hole filled with water which was pretty awesome. We had  good time there.
Take some snacks, and water. You may need them.
The post shows too many trails, but all of them keep coming as you drive up the hill. 
Better to start early as you have to be out of the forest by 6 PM. Again, by 6 PM you should be OUT of the forest, not that you can start back at 6 PM. 
 
If you have time and interest, I ask you to consider either Culebra or Vieques. Both are islands which take an hour on the ferry. The ferry terminal itself is an hour from the main area. 
Going to the islands is time consuming and the beaches are well well worth going there. 
The biggest issue is, the ferry tickets are not available online and you will have to go there and get them in person in line. The schedule is limited and if you want to do that, you should wake up first thing in the morning and do that. Again, the trip is well worth. That is why I wanted to do the Culebra Aquafari, because the tour company will get the ferry tickets for you and you will know for sure that you can get on the ferry.
Think of the ferry operations as limited RTC Bus Service in India and you get a correct idea. Hahahaha. 
I got sea sick on the way back. Better be prepared for choppy water.
 
Downtown was very very good. You would not need any pin pointing here, as things are self explanatory.
There are 2 forts. We did only one (del Morro). 
Depends on your personal taste, you can do from 1-4 hours. 
The fort itself is good but the views on the way to the fort, outside the 4th level, around the fort are awesome. It reminded me Jaipur forts with waterfront. Awesome
 
Downtown, you can spend so much time just walking the streets, looking at shops and stuff, eating. At one portion of the Old San Juan (aka downtown) there are Cruise docks where there could be some docked cruises. That area is more modern, with a small park, water front grill etc. It is great in the evening. 
 
For downtown, you would not need any tour in my opinion. We did not take any. Walk around and enjoy. 
 
There is food all over the place, but this is where we ate:
 
Casita Miramar. Really good. Very very good.
 
Kasalta
Too many options, normal place. Affordable and good. I liked it but not like die for it. Obama went there, so hyped up.
 
 
Verde Mesa, Downtown
Pretty Good. Veg friendly, but lots of meat options as well.
 
Marmalade
If you are in to fancy dining, you should make a reservation right away.
 
Budatai, Condado Area
Good thai food, both veg and meat options.
 
I cannot think of anything else.
 
In short:
Make reservations for Culebra aquafari if you are interested. Make reservation for Marmalade as well if you want to go.
Everything else, is do-able adhoc. 
Btw, I forgot that they have the worlds longest zipline which I did not do. Check that out as well.
 
If you need anything lmk. After all, you will be in US and your phones work well. 4G/3G works great as well.

San Juan downtown(1-2hrs), rainforest superb half day, flamengo and other beaches awesome. Try to explore more beaches.. Zip lines and climbing the best in the country (mountain-mountain), visit an island and do scuba and snorkel(half day). No visa required.
