h4 ead , Question 11, i765

Hi ,

I came to us on f1 visa, got my opt EAD and extension EAD. Later converted to  h4  status. I want to apply for H4 EAD. On question 11, it is asking for any previous EADs with location details  and dates.

Since I have 2 EAD's from my F1 status, should I declare both of them or just the latest?

Declare both. and mention it as OPT EAD or the EAD number ..they will track based on it..

If both, how do I present them on I765. There isnt' much space.

Uscis office: ( texas service center)

Dates: OPt start date, stemp opt start date

Results: granted : opt, Stem opt????
