Let me explain . Any insurance you will have deductible, co pay,  and out of pocket maximum.

 

lets say x has 300$ deductible, 10% co pay, 1500$ out of pocket maximum. 

 

If X got admitted in hospital. X will pay first 300$ deductible. After that he will pay 10% co-pay untill his total out of pocket maximum is 1500$ including the first 300$ he paid as deductible. Then every thing is 100% covered by insurance.
