Please find the attached notarized G-639 form.

3)you will recieve letters acknnowledging ur request.it will have a case number.DOL will email you this.For perm DOL will ask a authorization letter which you might need to notarize.

 

4)uscis will also send an email/letter in case of any further information needed.

What if we don’t have the 140 receipt number when requesting for it?
