can you please outline the steps on how to apply?

I know that i get to that link, but what did u write in Part3- 7th item in G-639 form. " Application,Petition or request Receipt number?" and also about Part 4- Section 8.a and 8.b do we need to simply sign or this requires notarized affidivit. If so where can it be done in United states?
