sanjaysahu Posted February 14, 2017 Report Posted February 14, 2017 same thing happened to me 3 yrs ago... 1) call u s ci s support line ..they will open a case 4 u ,make a note of it 2) unfortunately you have to go through same process...send all documents as you sent for applying opt including payment even though its their fault they will say its the process unless some thing changed recently 3) In addition to above documents , you may have to send the opt card back (make sure you keep a copy with u ) and a cover letter with case info and correction details 4) make sure to send the package to service center listed on the approval doc which came with your opt card (you may hav sent the original application to diff service center but this one has to go to service center listed on the approval doc which came with your opt card) 5) wait 1 week or so call customer service center and ask them to escalate to level 2 support...
