Question for experts F1H4

My wife has opt extension till mid September but she applied for change of status to h4/ead along with my h1 extension back in Feb and h1 and h4 ead got approved for 3 years in march with start date of 06/28. issue1: Her sevis did not get updated with change of status. From her school's prospective, She is still on F1. Question1: Is this really an issue? Since she has new i94. if this is going to be an issue, Who can fix sevis? How to fix sevis record? issue2: on May 30th (in F1 OPT status) she moved to new employer and did not inform university and did not get new i20 and didn't fill new i983. From June 28th she continued working for new employer with H4 EAD, For 4 weeks she worked for new employer in opt status without updating to university. Question2: Since she didn't inform university about new employer, Her sevis was not updated with new employer. what happens if there is no i20 with new employer?  Can university generate new i20 backdated? Will this going to be issue when she applies for H1 or when she goes for h4 stamping?

issue1:  h4 change of status i797 lo  strat date nunchi change of status vasthundhi ...  immediate ga kavali antay go to india and get stamped.
