When should the employment date start : When it was approved or when employer receives the hard copy of approval notice ? 

In the event if the employer says he did not receive the notice yet even after 2-3 weeks what should be done ? Employer should start paying me from day 1 right ?

It was through a mid-tier consulting company and he did not pay anything for the petition.

if this is new h1b picked in lottery last year, he may get approval from Oct 1st 2017 or may be from petition approved date.. it depends on adjudicating officer.

Dont worry about taxes..he need to get SSN first..

if you speak technically, he should be paid from the date he got approved which will never happen with desi employers..
