help regarding D S 160 questions

Hi all,

I have few questions filling d s 160, konchem help cheyyandi please. 

 is this the correct URL for booking visa appointment in Canada: https://ais.usvisa-info.com/en-ca/ 

1) Have you made specific travel plans? Intended date of arrival and intended length of stay? - ante already I am in US and inka return tickets book chesukoledhu. so approximate return dates ivochu kadha?

2) Have you ever been in US: provide information on your last five US visits? -- ante nenu US ki return vachina every time I have to give the details? 

---for example nenu 6 years in US lo so far unnanu anukunte and if I have traveled to India or any other country for vacation for few weeks, then do I have to include all the details? 

3) Are you applying in the same country or location where the visa above was issued, and is this country or location your place of principal of residence? -- my last visa was issued in Canada but that is not my place of principal of residence kabatii "NO" kadha answer.

4) Have you been ten-printed? naaku sariga gurthu ledhu I think "NO"

5) US point of contact: naa sibling details isthe emanna problem aa (ante idharu Ikkade unnaru ani  antara?)

6) Do you have any immediate relatives, not including parents, in the United States?  naa sibling details isthe emanna problem aa (ante idharu Ikkade unnaru ani  antara?)

7) Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services? ante GC process aa ? I-140 apply chesthe "Yes" aa

DS-160 ani google cheyi immihelp lo or copy paste the same question in google u will get a sample explaining how to answer each and every question
