I got Employee - Employer Relationship Rfe and below are the documents that i submitted and it got approved

Monthly Periodic Review with achievements, issues, tasks completed or in progress

Weekly Time Sheets with 4 or 5 bullet points what i had done that week.

Client and Vendor letter, with end date (doesn't matter if they give for 6 mons or 1 year) But ask them to include that possibility of further extension.

Client Organisation Chart showing to whom you report

Project description in two page documents with you're roles and responsibilities explaining in detail.

Any Appreciation emails in a single pdf. 

 

That's it, its a simple query and it will approve easily. Good luck.
