There will never be pilot error with oxygen supply , It could be technical fault , again , if the oxygen fails you can bring the airplane below 10000 feet's , as we go up our atmosphere pressure will go down and we are in pressurized cabin . 

He might have brought the airplane down to help with the atmospheric pressure below 10 k , that might have caused some trauma from air pressure changes. Usually pilot mistakes happen when they don't follow protocol , again flying is the most safest mode of transportation ( i am a  private pilot) 

The kid oxygen is different from the whole airplane oxygen , i am certain , there should some or other doctors in airplane , but the pilot should have made the decision to land the plane if notified about emergency , if he didn't than that would be a negligence ,
