All of these have completed residency and to work in profit based hospitals or start clinics u have to take H4 EAD as they cant start clinic business on H1. These clinics are in Medically Underserved Areas and not in metro cities. So they have potential upto 1 million USD. My wife's seniors started near Missisippi etc (Delta region: DRA). So they will have to close clinics & literally many such MUA's are good for rural population because Americans dont go those areas.. J1 waivers are usually done in these areas because the authorities clearly mentions that (DRA, Conrad 30) they are not taking American Jobs. 30k-40k people u see in walmart are US citizens (family based) and I did ask some in Walmart & CVS is New Jersey ...That;s just incompetent H1's rumors...The Avg salary for H4 EAD is 76k.
