You need to pay $85 biometrics fees and USCIS will send you a window time to go the center and provide biometrics and once this biometrics is approved, then USCIS will proceed with the extension of the application. Earlier, people used to apply for H4 extension along with H1B and if they filed in premium, they used to get it along with H1B extension. Quote

We seek to force USCIS through litigation to adjudicate these renewals in a reasonable time as required by the Administrative Procedures Act. We have yet to see any approval of an H-4 after the biometrics requirement began, even for those filed with a premium processing H-1B. This means H-4 EADs are being delayed.
