No it is not related to university. We need to submit all documents to USCIS

We get DS 2019 form from an agency who supervises foreign doctors and submit all relevant documents to USCIS

Hmm, I don't know. If she finished USMLE and three steps, applying for J1 should take 3-5 months. In some residency programs, if the University is ok, then she can apply for H1 and pay more to get it in 15 days. But she should finish 3 steps and have ECFMG certificate.
