I dont think you can apply extension when out of country. Asalu visa ayipoyelopla back vachi chesukodaniki em problem ? i140 is for post 6 yrs what does it have to do with applying extns or transfers other than that ? 

Asalu extn ki transfer ki quota enduku untundi?  If your visa expires and you are out of country appudu next april lo fresh apppication esukovali is what i told. Appudu you cant even use masters quota.  Malla lottery and all process same to same in general pool.

Visa stamping still valid vunte back normal ga enter avachu.

But, with you leaving US u will fall Out Of Status. So, will that H-1B stamping validity still legal???

Visa and I-94 both expired? - then it should be a fresh application. But should it go through lottery again? Or since I140 approved, can it be outside of quota?

What I-140 is withdrawn by Employer?

Once we got h1 ..no need to apply in general quota for extn until 6. years...even if we r in india....if have i140 aprroved we can do it beyond 6 yrs also...
