Birth certificate name addition and correction to parents name (485 filing)

I got Birth certificate from E-Seva and my name on the certificate is blank and my father name is spelled incorrectly. When I asked why my name is blank they said they got the record from hospital at the time of my birth and we don't provide name at that time so they left blank. They are saying it's not possible now to add name and they are waiting for Govt GO to update the records. What are my options now? We need Birth certificate for filing 485.

One afidavit with parents and Uncle sign ??? or All separate affidavits from each of us ?? like 1 affidavit from Parents to sign.. 1 affidavit from Relative/ Uncle?? good???
