I paid $140 for doctor fees. Rest all covered by insurance. Also make sure to send the sealed envelope within 2 months after doctor signature..else it will be invalid

At odcotr offcie they dont..Better go for cvs clinics or some small doctor office..they will cober under insuarance..

350 is good deal.If your PD is beyond 2011 dont even consider doing it..
