Because it is unregulated and messy and mostly operated by lower caste people and hence govt's rarely bother providing any infrastructure .

Even if the survey is wrong by a big margin it's stikl several times more infections than official numbers

People lack empathy towards the infected person. The infected person lacks responsibility to isolate and not to spread further. Everyone at fault
