i$85 filing medical new forms question

Hello friends,

I got my medicals done on sept 30 but looks like there was a new form that was released on october 1.

Are we supp to use new forms even though we got the medicals done on sept 30.

we got the medical results on oct 5 but the doc has not signed off on them.

Paatha form discard chesi kotha form ni back date chesi sign cheyamantunnaru.

Any one in the same boat? if so what did you guys do.

Ask your lawyer , they answer the best and latest

Use new forms. Doctor's signing date.

Your medical results are also available in Oct.

If signing date is after 10/1 - it should be on new forms.
