Born in a private Hospital, Birth Certificate

Hospital is still in place and they will give me a brith certificate if I request. Is that enough for I 485? or Do We need hospital records data etc?  Experts please help 🙏

certificate from govt and since it is past more than a year, you may need affidavit from parents/siblings I guess.

Birth certificate lo na last name just initial paddadi, passport lo correct undi, passport lo la marchamante maarusthara ee e-seva lo?.
