I have that white card given by CVS and Walmart .... is that sufficient while going from here and also while coming from India?

where do we need to upload while coming from india .... from here i understand airsuvidha form

This is correct , carry one copy of air suvidha filled form and one proof of vaccination .

If you are traveling to India from US, upload the vaccination proofs to air suvidha site. Print and carry the self declaration form, that would suffice.
