i did this for Andhra where only second page was filled... might be same for TG also.. address fill your parents indian address or any other indian address

I am from AP and so I think it is not applicable to you.  Anyways, it is form 32A. I did not do any notary at that time.

notary is not required...just send hard-copy of finger prints and signature with photos.. that's it
