it started as service where a person can offer direct service (live streams, shows, photos) as subscription basis to their fans...hence the name onlyfans.  they are just middleman...different from traditional porn sites. 

however, the platform need not be just for porn - they tried to bring legitimacy and tried to attract any artists to engage with fans. I don't know how well they are doing...if a person has onlyfans account, it is assumed that it is for either porn or sensual.
