this is the classic paper by James Parkinson in which he described 6 patients of Parkinson's disease with classic symptoms

although there are few mentions in other historical texts like charaka samhita where it is mentioned as kampavata which mentions the tremor aspect of it

also by greek philosopher Galen where he classified tremors

the main stay of treatment is Levadopa routinely administered along with Carbidopa , Levadopa is the most effective but it has side effects such unnecessary movements and Amantadine is used to reduce the side effects 

some other patients are started on Pramipexole and later started on Levadopa

this should be treated with great expertise in my view to handle the side effects 

Deep brain stimulation using electrodes to stimulate certain areas in brain like STN, GPi areas has met with success in few selected cases where there are severe tremors or severe side effects on Levadopa

gait training and exercise with full range of active and passive movements are proven helpful 

canes and walkers may be necessary to avoid falls

treatment is not simple like straight line ba there are many combination of drugs which varies with which is dominant symptoms and should be varied based on side effects depends on doctors discretion and experience

we haven't understood parkinson's disease enough, we know it affects dopamine producing neurons causing degeneration of those , thus leading to the symptoms, but haven't understood completely the insults that cause the degeneration , some genetic mutations are identified but it has environmental factors too such as exposure to pesticides and herbicides but no concrete evidences

some research has shown that aerobic exercise might reduce the risk of Parkinson's

caffeine products such as coffee, green tea are also related to reducing the risk

as said further studies should be done to get better picture

another useful sign of parkinson's disease is

when asked to draw a house , the lines of the door doesn't reach the floor which makes it look like it's floating

so this sign is called FLOATING DOOR SIGN

another useful and great sign for diagnosing parkinson's is ask them to draw a archimedes spiral

and another interesting sign is when you ask a normal person to whistle they smile after it but parkinson's patient fail to smile called whistle smile reflex
