******OPT Extension doubt...please advise********

Hi AFDB brothers,

I am applying for 17 month OPT Extension and I have a question on where ( which USCIS office) to send my application packet to ?

I did Masters in Texas, my employer is in Michigan and I am working in Arizona. So, to which USCIS should I send my packet? Which address can I provide on I 765 form.

I am confused on this....so please advise on this...........
