i would suggest go for multiple visa...and usually if you go for mutliple you would get the visa till your passport expiry date or H1b expiry whichever is sooner as per my knowledge...

if your previous visa has been expired, then these guys ask you to get an appointment confirmation for H1B stamping(of course which u can later reschedule it, if you are not okay with the date...)...then only they will process it...
